Easy to Use California Advance Health Care Directive

This easy-to-read and easy-to-understand advance health care directive form was created for everyone, but in particular the 90 million American adults who are unable to read above a fifth grade reading level, many of whom struggle to understand the complex legal documents that are typically used to express how they want their health care handled if they become very sick. This form was developed by Rebecca Sudore, M.D., a physician at the University of California at San Francisco, in cooperation with the San Francisco General Hospital and the San Francisco Department of Public Health
